To enquire about a booking for your dog/s, cat/s or small pets, please email me or call me regarding your requirements; after which I will ask you to complete the relavent booking form (from the list below), thank you.
Dogs in season: please note 5 Star Pet Services cannot accept any female that is in season regardless of any previous booking made prior to the season commencing (see prices page for further information).
BOOKING FORMS REQUIRED: Anyone attending an induction will be required to bring with them the relevant paperwork (booking form, Medical and Feeding Guides where appropriate) if not the induction will be cancelled by 5 Star Pet Services in arrival at cost to yourselves, if you then wish to rebook you will be charged.
The paperwork is to give me information about your dog(s), your dogs vets should it require urgent medical treatment, all relevant contact numbers should I need them and information about your dogs character(s) and training levels etc it helps keep your pets safe.
The onus is on yourselves to print the relevant forms. We, will not print them for you, nor do we have copies for you to complete here.
